Role Responsibilities
As a Community Health Worker, you will collaborate with a dynamic team of healthcare professionals, including Nurses and LPNs, to support clients in their homes. Your role will involve providing essential home support services, focusing on personal care and daily living activities while promoting client independence.

Key Duties Include:
  • Assisting clients with daily personal care tasks such as bathing, dressing, grooming, and oral hygiene
  • Performing household tasks to maintain a safe and healthy living environment
  • Preparing meals according to established dietary plans
  • Providing assistance with mobility and transfers
  • Monitoring and reporting any changes in client behavior and condition
  • Executing delegated tasks in accordance with training and established procedures
This position is a permanent part-time opportunity with scheduled evening shifts.
