Your Opportunity:
This position reports to the Home Care Manager, in Alberta. As a Home Care Registered Nurse Case Manager, you will provide case management services and interventions to clients with a variety of healthcare needs. Your responsibilities will include the oversight and coordination of healthcare needs of clients across the lifespan, including chronic disease management, health assessments, assessment for assistance with daily living activities, wound assessments, wound care, assessments for placement in long-term care facilities, as well as palliative and end-of-life care.
About the Role:
As a Registered Nurse (RN), you will provide a wide range of nursing services to patients, families, communities, and populations, while ensuring their safety and well-being. You will utilize nursing processes, critical thinking, problem-solving, and decision-making to provide high-quality patient care. You will also teach, counsel, and advocate on behalf of patients and their families, playing a leadership role in supporting an integrated and holistic approach to patient care, health promotion, and maintenance.
Requirements:
- Completion of an accredited nursing education program
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)
- Completion of the CRNA Medication Management - Chapters and the NextGenU Health Assessment Resources Certification (if not practiced as a Registered Nurse within the last five years)
Preferred Qualifications:
- A Master's Degree in nursing or a related field
- Aids to daily living (AADL) authorization experience
- 1-3 years of Home Care Case Management experience
