Your Role as a Licensed Practical Nurse

As a Licensed Practical Nurse at Hope to Health Primary Care, you will play a vital role in delivering high-quality patient care. You will work closely with physicians, nurses, and other healthcare professionals to provide comprehensive nursing services, ensuring the well-being of our patients.

Key Responsibilities:
  • Provide direct patient care, including medication administration, wound care, and vital sign monitoring.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Educate patients and their families on health conditions, treatment options, and self-care strategies.
  • Document patient care accurately and thoroughly in electronic health records.
  • Respond effectively to emergencies and critical situations.
  • Participate in quality improvement initiatives to enhance patient safety and outcomes.
Qualifications:

To be successful in this role, you will possess:

  • A valid license as a Licensed Practical Nurse.
  • Demonstrated clinical skills and experience in a primary care setting.
