Job Summary
Vancouver Coastal Health is seeking a Community Health Worker to join the Home Support Team in Squamish, BC.
As a Community Health Worker, you will provide home support services to clients, working under the direction of the Supervisor/Coordinator/Manager or delegate. You will assist clients with varied levels of care, observe, and report significant changes in the client's presentation.
Key Responsibilities
- Provide home support services to clients, including assistance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
- Collaborate with the healthcare team, clients, and their caregivers to maximize independence.
- Motivate and encourage clients to participate in their care.
- Provide input and feedback in the care planning process.
Qualifications
Education & Experience
- Graduation from a recognized Health Care Assistant Program.
- Current/Active Registration with BC Community Health Worker and Care Aide Registry.
- Experience interacting with persons with dementia, geriatric, and/or palliative population will be an asset.
Knowledge & Abilities
- Basic knowledge of client-centered care and promoting independence.
- Demonstrates correct use of body-mechanics and knowledge of injury prevention.
- Knowledge and ability to operate mechanical lifts, following safe work procedures.
