About the Role

We are seeking a skilled Social Worker to join our team at Bruyère Continuing Care. As a Social Worker, you will play a vital role in providing patient-centered care and support to individuals and families in our community.

Key Responsibilities
  • Provide social work interventions to patients and families, addressing their emotional, psychological, and social needs.
  • Collaborate with healthcare teams to develop and implement care plans that meet the unique needs of each patient.
  • Conduct assessments and develop strategies to address social determinants of health, such as housing, employment, and education.
  • Provide counseling and support to patients and families, addressing issues related to grief, loss, and trauma.
  • Develop and maintain relationships with community resources and services to support patients and families.
Requirements
  • Master's degree in Social Work (MSW) from an accredited university.
  • Membership in the Ontario College of Social Workers and Social Service Workers.
  • Fluent in French and English.
Preferred Qualifications
  • Knowledge of community resources and services.
  • Experience working in a hospital setting or with patients with complex medical conditions.
  • Experience with discharge planning and patient advocacy.
