Job Title: Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Fraser Health. As a Clinical Pharmacist, you will play a vital role in providing clinical pharmacy services to our patients.

Key Responsibilities:
  • Provide individualized pharmacotherapy to patients
  • Counsel patients on their medication therapy
  • Work collaboratively with physicians and other healthcare providers to develop and implement therapeutic plans
  • Assist in the development of drug therapy guidelines for individual disease conditions
  • Prepare and submit written summaries and recommendations on drug usage to the Pharmacy & Therapeutics Committee
  • Contribute to the development of case studies and articles for submission to professional journals
  • Act as a department resource person in the assigned area of responsibility
  • Participate in medication use evaluations and clinical drug trials
  • Provide instruction and/or supervision to pharmacy students and residents
Requirements:
  • Current registration as a Pharmacist with the College of Pharmacists of British Columbia
  • Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience
  • Broad knowledge of organization policies, procedures, and standards of care
  • Knowledge of evidence-based clinical pharmacy practice
  • Knowledge of medication use evaluation
  • Knowledge of research processes and methodology
  • Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision
  • Ability to communicate effectively both verbally and in writing
  • Ability to establish and maintain effective working relationships with other healthcare practitioners, patients, family members, and the public
