We are seeking a highly skilled and dedicated Occupational Therapist to join our dynamic Rehabilitation Team at CarePartners. As a key member of our team, you will play a critical role in providing high-quality patient care and contributing to the growth and development of our therapy and rehabilitation program.
Key Responsibilities- Provide professional assessment and treatment of patients in the community and in their homes
- Develop and maintain effective care plans in collaboration with the Home and Community Care Support Services (HCCSS)
- Work with patients to establish and achieve time-specific, measurable goals and outcomes
- Provide guidance and supervision to Community Support Workers in the field
- Participate in interdisciplinary care teams and contribute to the development of care plans
- Perform other duties as required
- Master's Degree (or equivalent) in Occupational Therapy from a recognized university
- Registration with the College of Occupational Therapists of Ontario
- Proven computer skills, including email, MS Word, and Outlook, work scheduling systems
- Strong assessment skills and ability to participate in effective treatment programs
- Excellent English verbal and written communication skills
- French language skills are an asset
- Clear Background and Vulnerable Sector Check
