YOUR ROLE IN CARE – A PLACE TO THRIVE
Under the guidance of the Patient Services Manager, the Admissions Coordinator plays a crucial role in managing and authorizing both long-term and short-term admissions to Long-Term Care Facilities for individuals from various sources, including community members, hospitals, and other care homes.
Key Responsibilities
- Address inquiries from both internal and external stakeholders regarding placement regulations and processes.
- Oversee, validate, and maintain applications for Long-Term Care Facilities within the designated region and beyond.
- Collaborate with clients, families, and both internal and external partners to ensure optimal outcomes during the placement journey.
- Keep track of waiting lists and available spaces in Long-Term Care Facilities.
- Recommend suitable candidates for bed vacancies in Long-Term Care Facilities.
- Serve as a resource for partners and committees by providing updates on application statuses and identifying potential challenges.
- Ensure all aspects of the application process are documented in compliance with regulatory standards.
- Gather and analyze statistics for management reporting as needed.
Qualifications Required
- Post-secondary education in health, social sciences (e.g., Psychology/Sociology), or a related social services field.
- A minimum of two years of recent experience in a relevant health or social services role.
- Familiarity with long-term care admission processes is advantageous.
- Strong understanding of applicable legislation and regulations.
- Self-motivated with the ability to organize, prioritize, and manage multiple tasks effectively.
- Proficient in analyzing information, problem-solving, and making informed decisions.
- Detail-oriented with strong documentation skills.
- Excellent communication skills to facilitate effective information exchange.
- Strong customer service and conflict resolution abilities.
- Proficient in MS Office applications (e.g., Word, Excel, Outlook, PowerPoint).
- Proficiency in French or another language is a plus.
