Care Manager

At Stay at Home Nursing, we're dedicated to delivering exceptional home healthcare services that empower individuals to achieve their health goals. As a Care Manager, you'll play a pivotal role in shaping the future of home care, working closely with our interdisciplinary team to craft personalized care plans that drive positive outcomes.
Key Responsibilities:- Client Care Management & Experience:
- Develop comprehensive care assessments and updates for every client
- Ensure seamless caregiver-client introductions and relationships
- Conduct supervisory visits to guarantee high-quality care
- Collaborate with clients' families and healthcare teams for continuity of care
- Educate clients on self-care and adapt care plans based on risk factors and outcomes
- Caregiver Leadership:
- Lead and inspire your caregiving team through coaching, education, and goal-setting
- Ensure caregivers provide exceptional care leading to client satisfaction
- Oversee staff performance, adhering to policies and best practices
- Manage onboarding and training programs for a continuous learning environment
- Scheduling Collaboration:
- Collaborate with the scheduling team for timely and accurate client file handoffs
- Communicate changes in client needs and caregiver assignments
- Assist with scheduling to ensure client coverage, especially in urgent situations
- Maintain compliance with clock-in/clock-out procedures
- Consult Conversion:
- Conduct client consultations, assessing needs holistically and achieving an 80% conversion rate
- Build relationships with community leaders and organizations for referrals
- Provide expert advice during consultations
- Administration:
- Maintain accurate documentation of care plans, employee files, client records, pay, and billing information
- Review and update client records regularly
- Conduct quarterly audits to maintain documentation standards
- Ensure compliance with national and provincial health, employment, and safety regulations
- Nursing Diploma and CNO Registration Required
- Nursing background (RN or RPN) required
- Reliable vehicle and valid driver's license
- 1-2 years of nursing experience preferred
- Strong computer skills, including Office 365 proficiency
- Excellent written and verbal communication skills
- Highly organized, with the ability to manage multiple tasks and set clear goals
- Customer service and leadership experience considered an asset
This is an in-person office job, Monday to Friday, 9:00 AM - 5:00 PM.
