About the Role

Vancouver Coastal Health is seeking a skilled Social Worker to join our team at Powell River Hospital. As a Social Worker with our organization, you will play a vital role in providing comprehensive social and psycho-social assessments, collaborating with clients and interdisciplinary teams to facilitate transitions, and providing individual, family, and group counseling services.

Key Responsibilities

  • Conduct thorough social and psycho-social assessments to identify client needs and develop effective care plans.
  • Collaborate with clients, families, and interdisciplinary teams to ensure seamless transitions and optimal client outcomes.
  • Provide individual, family, and group counseling services to address a range of mental health and social issues.
  • Attend clinical and case conferences to review client progress, admissions, and discharges.
  • Provide consultative and direct services to clients, families, staff, and community health professionals on issues such as adult guardianship, advanced directives, domestic violence, abuse and neglect, immigration, housing, financial concerns, and government policies.
  • Prepare and maintain accurate client records in accordance with established standards, policies, and procedures.

Qualifications

  • Master's Degree in Social Work from an accredited School of Social Work.
  • Three years of experience working in a psychiatric setting, providing treatment services to individuals with complex mental health, addiction, and chronic medical concerns.
  • Valid BC Driver's License.
  • Current full registration with the British Columbia College of Social Workers.

Knowledge and Abilities

  • Knowledge of the principles of recovery in mental illness.
  • Knowledge of Psychosocial Rehabilitation methodology.
  • Knowledge of the Mental Health Act and its application to client rights and staff obligations.
  • Knowledge of community, social, and mental health resources.
  • Ability to identify emotional, social, or environmental barriers and intervene to eliminate stigma.
  • Ability to effectively manage conflict situations and aggressive behavior, using non-violent crisis intervention and behavioral management techniques.
  • Ability to work independently and collaboratively as a member of an interdisciplinary team.
  • Ability to communicate effectively, both verbally and in writing, with clients, families, team members, and community agency personnel.
  • Broad knowledge of mental illness and addictions treatment.
  • Broad knowledge of systems thinking as it relates to social work practice.
  • Broad knowledge of other health disciplines and their role in healthcare.
  • Broad knowledge of research methodology, counseling skills, and clinical practice models.
  • Broad knowledge of family, group, and change therapy.
  • Broad knowledge of legislation and government policies and procedures related to clients.
  • Demonstrated ability to apply systems theory to practice in social work assessment and intervention.
  • Demonstrated ability to conduct social and psycho-social assessments.
  • Demonstrated ability to understand and influence group and inter-group processes, decision-making, and problem-solving.
  • Demonstrated ability to establish rapport with clients and families.
  • Demonstrated ability to adjust to new or unexpected events and deal effectively with conflict.
  • Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
  • Basic understanding of trauma-informed care.
  • Basic computer literacy to operate a computerized client care information system and word processing, spreadsheet, Internet, and e-mail software.
  • Physical ability to perform the duties of the position.
  • Skilled in the use of equipment and techniques appropriate for treatment.
