Job Title: Mental Health Support Worker

We are seeking a dedicated and compassionate Mental Health Support Worker to join our team at Chezranti Community Centre Ltd. As a key member of our community, you will play a vital role in providing support and guidance to individuals in need.

Key Responsibilities:
  • Case Management: Assess client needs, develop and implement individualized plans, and provide ongoing support to ensure successful outcomes.
  • Counseling and Education: Conduct individual and group counseling sessions, provide education on life skills, and facilitate workshops to promote healthy behaviors.
  • Community Outreach: Establish relationships with community partners, provide referrals, and facilitate connections to resources and services.
  • Documentation and Reporting: Maintain accurate records, prepare reports, and contribute to program evaluation and research.
Requirements:
  • Education: College/CEGEP diploma in a related field.
  • Experience: 2 years to less than 3 years of experience in a related field.
  • Skills: Excellent communication, interpersonal, and problem-solving skills; ability to work independently and as part of a team.
