Job Title: Social Worker, BSW

Job Summary:

We are seeking a highly skilled and compassionate Social Worker, BSW to join our team at Fraser Health. As a key member of our interdisciplinary team, you will provide psychosocial/emotional assessment and therapeutic clinical counselling services to clients in our assigned clinical areas.

Key Responsibilities:

  • Conduct psychosocial/emotional assessments of clients using various modalities and in accordance with professional standards and clinical policies and practices.
  • Provide basic therapeutic clinical counselling services to clients based on a psychosocial assessment.
  • Develop and implement care plans that address the psychosocial/emotional components of illness, injury, and disease.
  • Collaborate with clients, healthcare professionals, and community agencies to provide education and consultation on social work practice and area of clinical specialty.
  • Maintain accurate and comprehensive client records in accordance with established professional/organizational standards, policies, and procedures.
  • Participate in ongoing quality improvement initiatives, including program/practice evaluation, identifying opportunities for change, and assessing available resources and/or gaps in service.

Qualifications:

  • Bachelor's degree in Social Work from an approved school of Social Work.
  • Current full registration with the BC College of Social Workers.
  • Knowledge and understanding of psychosocial/emotional issues related to illness/injury/disease, and current social work treatment modalities used to ameliorate these issues.
  • Ability to communicate effectively verbally and in writing with clients, healthcare professionals, and community agencies.
  • Ability to work effectively in a multicultural environment, independently, and as part of a multidisciplinary team.
