We are seeking a skilled and compassionate Community Health Nurse to join our team at Fraser Health Authority. As a Community Health Nurse, you will play a vital role in providing high-quality care to adults with complex health needs in the community setting.
Key Responsibilities:- Conduct comprehensive assessments of clients to identify their health needs and develop individualized care plans.
- Provide direct care to clients, including health education, counseling, and support to promote independence and self-management.
- Collaborate with interprofessional teams to coordinate care and services, ensuring seamless transitions and optimal outcomes.
- Develop and maintain relationships with clients, families, and community resources to ensure comprehensive support and care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance client care.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One (1) year recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient care setting.
- Valid BC Driver's license and access to a personal vehicle for business-related purposes.
