Job Title: Care Planner

Job Summary:

The Ma Mawi Wi Chi Itata Centre is seeking a Care Planner to join our Wi Che Win team. As a Care Planner, you will provide holistic support services to our program participants, including our Youth homelessness population. Your role will involve working with participants to develop and implement individualized plans, providing support and guidance to help them achieve their goals.

Key Responsibilities:

  • Provide individualized support services to program participants, including home visits and crisis intervention.
  • Develop and implement individualized plans with participants, including goals and objectives.
  • Work collaboratively with the Wi Che Win team to provide comprehensive support services.
  • Liaise with collateral agencies to ensure participants receive necessary services.
  • Contribute to a positive and inclusive team environment.

Qualifications:

  • Youth Care Diploma or acceptable equivalent and related experience.
  • Experience in developing and implementing individualized plans.
  • Working knowledge of crisis intervention strategies.
  • Demonstrated experience working with Indigenous participants.
  • Valid Class 5 Drivers License.
  • Current Police/Child Abuse checks.
