Position Overview

As a vital member of an interprofessional health care team, the Community Health Worker II operates under the guidance of a Team Lead or designated clinical leader. This role involves delivering home support services tailored to a pre-defined care plan. Responsibilities include aiding clients with daily living activities such as bathing, grooming, and meal preparation, all while fostering maximum independence. Additionally, you will perform delegated tasks for which you have received appropriate training. A core aspect of this position is to ensure the well-being of clients and their families, promoting a lifestyle of independence within the community.

Work Schedule

Shifts will vary, including Days and Evenings from 07:00 to 15:33 and 13:00 to 22:03, on a rotating basis.

Qualifications
  • Completion of a recognized Home Support Attendant or Resident Care Aid Program.
  • Registration with the BC Care Aid & Community Health Worker Registry.
  • Possession of a valid Class 5 BC Driver's License.
  • Certification in CPR, First Aid, and Food Safe.
  • Access to a personal vehicle.
  • Or an equivalent combination of education, training, and experience.
Skills and Competencies
  • Physical capability to fulfill the responsibilities of the position.

  • Skill in observing and identifying changes in clients' conditions.
