Job Title: Clinical Dietitian

Job Summary:

The Sioux Lookout Meno Ya Win Health Centre is seeking a skilled Clinical Dietitian to join our team. As a Clinical Dietitian, you will be responsible for assessing, educating, and treating patients who require nutrition support. This includes collaboration with the interdisciplinary care team to develop and implement effective nutrition care plans.

Key Responsibilities:

  • Screen, prioritize, and process client referrals using information obtained through the physician, laboratory reports, and health records.
  • Complete nutrition and health assessments to develop nutrition care plans with clients by integrating information about the client's health status and available community resources.
  • Develop, implement, and evaluate in-hospital diabetes management and facilitate ongoing outpatient follow-up.
  • Be able to identify and prioritize complex health and social issues.
  • Evaluate nutritional status of individuals and aid in the prevention and/or treatment of inadequate nutrition.
  • Perform basic bedside swallowing assessments and make diet texture recommendations.
  • Provide direction to the Manager Food & Nutrition Services and the multidisciplinary team to meet the nutritional needs of residents, including regular menu review and approval.
  • Monitor and evaluate the nutritional status of each resident at William George Extended Care, including their current nutritional risk, prioritizing as required; documents these reviews as per MOHLTC standards and regulations.
  • Ensure complete, accurate, and timely documentation of client records.
  • Communicate information clearly and concisely to clients and healthcare providers regularly.
  • Act as a nutritional resource person for clients, their families, other health professionals, and support agencies.
  • Assume responsibility for own development and maintenance of clinical competence.
  • Ensure safety is a priority.
  • Maintain accurate, comprehensive statistics. Maintain relevant records, files, and charts with pertinent nutritional information.
  • Maintain a research-based practice and participate in research projects, as appropriate.

Requirements:

  • Bachelor's Degree in Applied Nutrition or equivalent.
  • Completion of Dietetic Internship.
  • Registration with the College of Dietitians of Ontario, or eligibility to obtain.
  • Certified Diabetes Educator (CDE) required, or willing to obtain.
