We are seeking a highly skilled Clinical Nurse Specialist to join our team at Queensway Carleton Hospital. As a key member of our healthcare team, you will play a vital role in providing expert care to patients with diabetes.
Responsibilities:- Develop and implement comprehensive care plans for patients with diabetes, incorporating the Chronic Care Model to improve clinical outcomes.
- Provide expert nursing services, including health promotion and preventive care, to empower patients and families to manage their diabetes.
- Collaborate with interdisciplinary teams to facilitate continuity of care and improve patient outcomes.
- Participate in program planning and coordination to ensure seamless transitions of care and effective discharge planning.
- Design and deliver specialized education programs for patients, families, and healthcare professionals to promote quality care and improve patient outcomes.
- Masters Degree in Nursing (candidates with a masters degree in progress will be considered)
- Bachelors of Science Nursing Degree
- Minimum of five (5) years recent and relevant clinical nursing experience, including medicine, and/or chronic disease prevention and management experience
- Minimum three (3) years recent and relevant diabetes management experience in the context of other chronic conditions
- Registered with the College of Nurses of Ontario
- Certified as a diabetes educator
- Knowledge and experience with patient self-management support
- Proven ability in independent and collaborative decision making
- Strong communication skills – oral, written, electronic
- Ability to work with adult learners and colleagues in a variety of settings using a collaborative approach
- Member of the Registered Nurses Association of Ontario
- Member of the Diabetes Educator Section of the Canadian Diabetes Association
- Canadian Nurses Association (CNA) and/or related certifications
- Experience with clinical documentation in an Electronic Health Record
