Key Responsibilities
The Nurse Practitioner operates independently, delivering comprehensive patient care through advanced nursing practices and fundamental medical management skills tailored to a specific patient demographic.
In this role, you will manage care for patients with complex medical needs and provide end-of-life care in their residences. You will coordinate with patients, families, and healthcare providers, utilizing your clinical assessment and management expertise to ensure optimal patient outcomes. Your responsibilities will include assisting patients in pain and symptom management, preventing unnecessary hospital admissions, and addressing acute episodes of complex illnesses.
Position Type
This is a Regular Part-Time (0.5) position within a Rapid Response Team.
Essential Qualifications
- Active membership with the College of Nurses of Ontario
- Registered Nurse in the Extended Class (RN EC designation)
- Familiarity with community resources and the roles of healthcare professionals
- Review and confirm discharge care plans and ensure necessary medical tests and transportation are arranged
- Conduct medication reconciliation and discuss medication protocols with patients and their caregivers
- Initiate communication with primary care physicians to update them on patient care events and post-discharge plans
- Identify patients needing expedited assessments and collaborate with Care Coordinators for home assessments
- Support Care Coordinators in developing patient care plans and ensuring seamless transitions to ongoing care teams
- Address any inquiries or concerns from patients and caregivers
- Act as a representative when necessary, explaining the role of the Local Health Integration Network (LHIN) to patients and healthcare professionals
- Ensure a safe environment for patients, caregivers, and staff, adhering to health and safety policies established by the LHIN
- Participate in the establishment and monitoring of Case Management standards, contributing to quality improvement initiatives
- Other related responsibilities may be assigned.
Preferred Qualifications
- Specialized training in palliative pain and symptom management
- Experience in Palliative/End of Life Care
- Knowledge of Medical Assistance in Dying legislation
- Experience in managing complex chronic diseases and psychogeriatric care
- Advanced assessment capabilities and strong understanding of clinical therapeutics
- Demonstrated leadership and collaborative practice with all healthcare providers
