Key Responsibilities- Oversee the development, coordination, execution, and assessment of the PATH (Priority Assistance to Transfer Home) program.
- Engage in client discharge planning in collaboration with hospitals and community support services.
- Authorize referrals, conduct intakes, and ascertain care coordination needs, whether internal or external.
- Manage service/transportation requests and confirm acceptance of referrals with participating hospitals.
- Ensure adherence to safety protocols and policies established by the agency and its partners.
- Conduct follow-ups with PATH clients within 48 hours post-service delivery to finalize services, evaluate additional needs, and gather client satisfaction feedback.
- Assist in the preparation and monitoring of program-specific budgets, compile statistical reports, and contribute to progress reporting.
- Identify and establish qualitative and quantitative metrics necessary for program evaluation in consultation with hospitals, HCC, and partner agencies.
- Actively participate in the identification, development, and implementation of new or revised services and initiatives.
- A Bachelor's degree or a combination of one to three years of relevant experience and education.
- Fluency in English is essential; proficiency in French is an asset.
- Possession of a valid driver's license and access to a reliable vehicle for community travel.
- Proficient in Microsoft Office Suite (Word, Outlook, Excel, PowerPoint).
- Demonstrated experience in program management and development.
- Strong communication and presentation skills, along with leadership capabilities and experience working with clients and volunteers.
- Experience in administrative functions such as reporting, documentation, and scheduling.
- Standard First Aid and CPR-C certification is advantageous.
- Familiarity with community support services is preferred.
- The majority of tasks will be conducted in a clean and comfortable office setting, with occasional travel required within the designated area.
- Eligibility to work in Canada is required; candidates must ensure they meet the necessary visa requirements.
- Successful candidates will undergo a pre-employment screening process, including a satisfactory Enhanced Police Information Check (E-PIC).
- Full vaccination against COVID-19 is a requirement for this position.
Compensation Range: $43,746 - $45,000
