We are seeking a skilled and compassionate Neighborhood Health Team Lead Nurse to join our team at Bayshore HealthCare. As a key member of our community care team, you will play a vital role in providing high-quality, patient-centered care to our clients in the comfort of their own homes.
Key Responsibilities
- Build and maintain strong relationships with clients and their families, providing personalized care and support to meet their unique needs.
- Lead and manage a team of healthcare professionals, ensuring seamless care delivery and exceptional client outcomes.
- Develop and implement care plans that address the physical, emotional, and social needs of clients, in collaboration with our interdisciplinary team.
- Collaborate with other healthcare providers, community resources, and family members to ensure comprehensive care and support.
- Conduct regular assessments and evaluations to ensure the quality and safety of care, making adjustments as needed.
- Supervise and mentor staff, providing guidance and support to ensure they have the skills and knowledge needed to deliver exceptional care.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies.
- Stay up-to-date with industry developments, best practices, and regulatory requirements, applying this knowledge to improve care delivery and client outcomes.
Requirements
- Current registration with the OIIQ or OIIAQ, with a valid Nursing License.
- At least five years of recent experience in nursing, preferably in a community setting, with progressive responsibilities.
- Strong leadership and management skills, with experience in supervising and mentoring staff.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and healthcare professionals.
- Ability to work independently and as part of a team, with a strong focus on collaboration and teamwork.
- Geriatric care, community health, or social work experience is an asset.
