**Job Summary:**
We are seeking dedicated and compassionate individuals to join our team as Continuing Care Assistants, Personal Care Workers, or Personal Support Workers. **Responsibilities:**
* Provide high-quality care and support to clients in their homes or community settings
* Develop and maintain strong relationships with clients, families, and community partners
* Collaborate with our team to ensure seamless care coordination and support
* Participate in ongoing education and training to enhance your skills and knowledge
**Requirements:**
* Certification as a Continuing Care Assistant, Home Health Provider, Personal Care Worker, or Personal Support Worker
* Excellent interpersonal and communication skills
* Ability to work independently and as part of a team
* Valid criminal record check and vulnerable sector check
