Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Island Health. To be successful in this role, you will need to be licensed by, or eligible for licensing by, the College of Pharmacists of British Columbia.
In addition to your licensure, you will require completion of an accredited pharmacy residency and three years of recent pharmacy practice experience, including community-based practice or an equivalent combination of education, training, and experience.
Key Responsibilities- Identify and resolve drug therapy problems for elderly and palliative clients.
- Apply clinical knowledge and evidence-based decision-making to provide high-quality patient care.
- Establish and maintain effective working relationships with healthcare practitioners, care providers, clients, family members, volunteers, and the public.
- Communicate effectively both verbally and in writing to ensure seamless care coordination.
- Work independently, prioritize workload, and consistently complete tasks with accuracy, precision, and dependability.
- Teach and educate patients, families, and healthcare teams on medication management and other relevant topics.
- Be flexible and adapt pharmacy practice to a variety of community settings.
- Hold a valid BC driver's license.
