At Vision Loss Rehabilitation Canada, we are seeking a highly skilled and compassionate Client Care Navigator to join our team. As a key member of our healthcare team, you will play a vital role in coordinating and integrating supports and services around individuals living with vision loss.

Key Responsibilities:
  • Assess client needs and develop personalized care plans to achieve optimal outcomes.
  • Coordinate and facilitate linkages between clients, healthcare providers, and community services to ensure seamless care delivery.
  • Provide education and support to clients and their families to enhance their understanding of vision loss and available resources.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and address complex client needs.
  • Monitor and evaluate client progress, making adjustments to care plans as needed.
Requirements:
  • 3-5 years of experience in client services, preferably in the healthcare or social services sector.
  • Experience working with individuals with vision loss and their families.
  • Strong knowledge of community resources, healthcare systems, and clinical practices related to vision loss.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
  • Proficiency in technology, including Microsoft Office and case management systems.
Preferred Qualifications:
  • Registered health professional designation.
  • Minimum bachelor's degree in Social Work, Gerontology, or related healthcare discipline.
  • 3 or more years of experience working with individuals with vision loss.
