**Job Summary**

We are seeking a skilled Speech Language Pathologist to join our team as a Home Care Specialist. As a key member of our team, you will work independently with adult clients in their home settings, providing face-to-face therapy services and developing personalized care plans to improve their well-being.

**Key Responsibilities**

  • Conduct assessments in home settings to identify presenting impairments or diagnoses, using standardized assessment tools.
  • Develop and implement care plans based on evidence-based information, relevant best practice guidelines, and clinical expertise.
  • Collaborate with clients' family members, caregivers, and other healthcare professionals to ensure seamless care coordination.

**Requirements**

  • Graduate of a recognized post-secondary program in Speech Language Pathology.
  • Current active registration in good standing with the College of Audiologists and Speech-Language Pathologists of Ontario.
  • Valid driver's license and reliable vehicle.
  • Excellent communication and analytic skills, with strong organizational abilities.
  • Experience providing home-based services is an asset, but new graduates are welcome.
