Position Overview

The Nurse Practitioner (NP) will play a vital role within the Renal Program, reporting directly to the Clinical Director. As a Registered Nurse in the Extended Class (RN (EC)), the NP will be responsible for making clinical decisions and contributing to comprehensive care plans for patients suffering from chronic kidney disease (CKD). This position involves collaboration with Nephrologists, primary care providers, referral partners, and the community to facilitate smooth transitions for patients both home and to Nephrology outpatient clinics.


Key Responsibilities
  • Utilize advanced assessment skills and clinical knowledge to provide specialized care for patients with CKD.
  • Identify and address clinical practice challenges with a high degree of autonomy, managing both acute and chronic health issues while promoting wellness.
  • Collaborate with fellow Renal NPs to ensure comprehensive patient coverage across inpatient and outpatient settings.
  • Engage in interdisciplinary care by participating in team meetings and patient conferences, serving as a crucial link between patients and healthcare professionals.
  • Act as a mentor and resource for clinical nursing practice in CKD for the interdisciplinary team.
  • Maintain effective communication with primary care providers and community resources to ensure seamless patient transitions.
  • Provide nursing consultation to clients, families, and healthcare professionals regarding CKD management.
  • Contribute to decision-making processes that impact clinical practice at both the unit and program levels.
  • Lead and participate in the development of policies and procedures relevant to the renal patient population.
  • Engage in advanced practice initiatives, promoting education and research to enhance patient care.
  • Collaborate with other NPs across Nephrology Programs within the region.
  • Travel between various healthcare campuses may be required.

Qualifications
  • Master's degree in Nursing is required.
  • Current RN (EC) registration with the relevant nursing regulatory body is essential.
  • Certification as a nephrology Nurse (C.Neph) is required or a commitment to obtain certification within an agreed timeframe.
  • A minimum of 2 years of experience in caring for patients with CKD and related comorbidities is necessary.
  • Preferred experience of at least 2 years as a Nurse Practitioner.
  • Acute care nursing experience is considered an asset.
  • Strong understanding of chronic disease prevention and management is essential.
  • Knowledge of nursing functions and current healthcare system needs, particularly for CKD patients.
  • Exceptional decision-making, critical thinking, and problem-solving abilities.
  • Demonstrated organizational and leadership skills, with the ability to prioritize effectively.
  • Excellent negotiation and conflict resolution skills, with a proven ability to build relationships.
  • Self-motivated learner, eager to seek opportunities for professional development.
  • Ability to adapt positively to changing clinical situations.
  • Strong verbal and written communication skills.
  • Proven record of excellent attendance and punctuality.
