Job Title: Physiotherapist

Job Summary:

Home and Community Care Support Services North East is seeking a skilled Physiotherapist to join our multi-disciplinary team. As a Physiotherapist, you will be responsible for providing physiotherapy services to patients in their homes, schools, or community settings.

Key Responsibilities:

  • Provide physiotherapy services to patients to reduce, eliminate, or prevent physical disabilities/impairments and improve functional independence and safety.
  • Assess patients' physical abilities and develop individualized treatment plans.
  • Collaborate with healthcare professionals, case managers, and community organizations to ensure comprehensive care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care.
  • Stay up-to-date with relevant legislation, standards of practice, and professional guidelines.

Requirements:

  • Registered Physiotherapist in good standing with the College of Physiotherapists of Ontario.
  • In-depth knowledge of physiotherapy services and community resources.
  • Good understanding of healthcare professionals' roles and responsibilities.
  • Relevant physiotherapy experience in a community/healthcare environment.
  • Comprehensive knowledge of physiotherapy tools, processes, equipment, and assistive technology.
  • Knowledge of funding agencies related to physiotherapy recommendations.
  • Practical knowledge and understanding of relevant legislation.
  • Ability to prioritize professional duties, manage multiple patients, and efficiently organize workload.
  • Strong interpersonal and collaboration skills.
  • Effective listening, observation, and facilitation skills.
  • Ability to communicate information effectively through various means.
  • Working knowledge of computer software and Microsoft Office applications.
  • Valid driver's license and access to a vehicle.
  • Advanced oral and written proficiency in English is essential; French is considered an asset.
