Vancouver Coastal Health is seeking a skilled Physical Therapist to join their Complex Rehabilitation Support team at the Strong Rehabilitation Centre in Vancouver, BC.
Key Responsibilities- Work in partnership with clients and families to establish safe, realistic, and reasonable goals through assessment, planning, coordination, organization, implementation, evaluation, and transition of physiotherapy and other services.
- Assess service eligibility, facilitate the development, implementation, and adaptation of a care plan, deliver direct care, and provide care management to assist clients in managing their own care and navigating community services.
- Collaborate with other system partners and incorporate current evidence into practice.
- Current full registration with the College of Physical Therapists of BC (CPTBC).
- Recent experience in Home Health clinical practice in Community.
- Valid BC Driver's License required for local area travel.
