Job Summary:
We are seeking a highly skilled and compassionate Registered Practical Nurse to join our team at Mon Sheong Foundation. As a Registered Practical Nurse - Geriatric Care Specialist, you will play a vital role in providing high-quality care to our residents, promoting their physical, emotional, and social well-being.
Key Responsibilities:
- Assist and support Registered Nurses and other members of the Interdisciplinary Resident Care Team in planning, implementing, and evaluating resident-centered care models.
- Provide leadership and supervision to non-regulated staff, ensuring they are equipped to provide high-quality care and services to residents.
- Develop and maintain effective relationships with residents, families, and healthcare professionals to ensure seamless care coordination.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in geriatric care.
- Collaborate with the interdisciplinary team to identify and address resident care needs, ensuring a comprehensive and person-centered approach to care.
- Provide education and training to non-regulated staff on resident care and services, promoting their professional development and growth.
- Contribute to the development and implementation of care plans, ensuring they are resident-centered and aligned with the organization's goals and objectives.
- Monitor and evaluate the quality of care provided, making recommendations for improvement and implementing changes as needed.
- Participate in resident care conferences, home committees, and task forces to ensure effective communication and collaboration among team members.
- Maintain accurate and up-to-date records of resident care, ensuring compliance with regulatory requirements and organizational policies.
- Provide input on the development of policies and procedures related to resident care, ensuring they are aligned with best practices and regulatory requirements.
Requirements:
- Current certificate of competence from the College of Nurses of Ontario.
- Valid CPR (Health Care Provider Level C/Basic Life Support) and First Aid certificates.
- Minimum of one year experience in geriatric, rehabilitation, or long-term care.
- Ability to work as a team leader and demonstrate supervisory skills.
- Proficiency in oral and written English.
- Cultural sensitivity and ability to speak Cantonese/Mandarin in addition to English would be an asset.
- Relevant computer literacy.
