LEAD Program Overview:

The LEAD (Let Everyone Advance with Dignity) initiative is a collaborative effort designed to enhance community capacity for improving public health and safety while reducing the dependence on law enforcement and the judicial system for addressing public order challenges associated with drug use and sex work.

Role Summary:

The Outreach and Screening Coordinator serves as the primary contact for all referrals to the LEAD case management system. This role is essential for ensuring effective communication and collaboration among all stakeholders involved in the referral process. Key responsibilities include:

  • Acting as the main point of contact for all LEAD referrals.
  • Conducting initial screenings for new referrals and addressing immediate client needs.
  • Engaging in street outreach to connect with individuals referred to the LEAD program.
  • Coordinating the assignment of new referrals to appropriate case managers.
  • Building and maintaining positive, collaborative relationships with LEAD partners and other service providers.
  • Documenting all activities in accordance with agency and contractual standards.

Qualifications:

A high school diploma or equivalent is required, with an undergraduate degree preferred. Relevant experience may be considered in lieu of a degree.

Skills and Knowledge:

  • Proven experience in providing street-based outreach and engagement services.
  • Understanding of substance use disorders and harm reduction methodologies.
  • Strong interpersonal and communication abilities.
  • Excellent organizational skills with the capacity to prioritize tasks effectively.

Physical Requirements:

Employees may encounter exposure to illicit substances and fumes. The role may require occasional lifting or moving of items weighing up to 30 pounds.
