Position OverviewVancouver Coastal Health (VCH) is seeking skilled Registered Nurses to serve as Community Health Nurses within the Home Care Nursing division in Richmond, BC.
Key Responsibilities:
- Provide care for clients experiencing acute, chronic, and palliative health challenges.
- Collaborate with clients and their families to set safe, realistic, and achievable health goals through comprehensive assessment, planning, coordination, organization, implementation, evaluation, and transition of nursing and related services.
- Evaluate service eligibility, facilitate the creation and adaptation of care plans, deliver direct nursing care, and manage care to empower clients in navigating community services.
- Engage with other healthcare partners and integrate current evidence into nursing practice.
Education & Experience:
- Active registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- A minimum of one year of recent clinical experience in a relevant field, particularly with complex clients facing chronic health issues, or an equivalent combination of education and experience.
- A valid BC Driver's License is required for local travel.
Knowledge & Skills:
- In-depth understanding of community health nursing principles and practices within a client-centered care framework.
- Ability to provide trauma-informed, culturally safe care and conduct thorough client assessments.
- Strong critical thinking skills to assess safety and risk in various home environments.
- Effective problem-solving capabilities to address psychosocial and behavioral challenges.
- Proficient in managing multiple clients while fostering collaboration and innovation.
- Resilience in adapting to unexpected changes while maintaining a focus on client-centered care.
- Ability to work effectively within an interdisciplinary team to assess and manage diverse health challenges.
- Competence in documenting client information accurately in electronic medical records.
- Commitment to maintaining confidentiality and privacy of client information.
- Strong communication skills to build trust and rapport with clients, families, and healthcare professionals.
- Experience in teaching and coaching clients and families in self-management of care.
- Familiarity with provincial regulations and community resources related to health services.
- Knowledge of quality improvement processes and methodologies.
- Demonstrated organizational skills to prioritize and manage workload effectively.
- Physical capability to perform nursing duties and use relevant medical equipment.
