Job Title: Occupational Therapist

We are seeking an experienced Occupational Therapist to join our team at Home and Community Care Support Services. As an Occupational Therapist, you will play a vital role in providing patient-centered care to individuals in their homes and communities.

Job Summary:

The Occupational Therapist will be responsible for assessing patients' abilities and developing personalized treatment plans to maximize their independence and safety in their environments. You will work collaboratively with healthcare professionals, patients, and caregivers to deliver effective and efficient care.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' abilities and develop treatment plans to address their needs.
  • Provide occupational therapy services to patients in their homes and communities, including assessments, interventions, and education.
  • Collaborate with healthcare professionals, patients, and caregivers to ensure seamless care delivery.
  • Document patient information and treatment plans in accordance with organizational policies and standards.
  • Participate in quality improvement initiatives to enhance patient care and services.
Requirements:
  • Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario.
  • Experience working with diverse patient populations and healthcare teams.
  • Strong communication and interpersonal skills to work effectively with patients, caregivers, and healthcare professionals.
  • Ability to work independently and as part of a team to deliver patient-centered care.
  • Knowledge of occupational therapy principles, practices, and standards.
