Position Overview

As a vital member of the healthcare team, the Pharmacist will collaborate with an interdisciplinary group of healthcare professionals to deliver patient-focused care that enhances the safety, efficacy, and appropriateness of medication therapy. This role encompasses involvement in various medical services, identifying and addressing medication-related issues, conducting medication reconciliations, participating in interprofessional medical rounds, providing patient education, and offering therapeutic recommendations to the healthcare team.

Key Responsibilities

  • Engage in collaborative practice with family health teams and chronic disease management teams.
  • Oversee and ensure the safe and effective distribution of medications in both inpatient and outpatient settings.
  • Contribute to the achievement of the objectives and goals of the Pharmacy Services.

Qualifications

The ideal candidate will possess:

  • A degree from an accredited Pharmacy program.
  • Completion of a Pharmacy Residency Program is preferred.
  • Licensure to practice Pharmacy in the relevant province.
  • A minimum of two years of clinical experience in a hospital pharmacy environment is preferred.
  • Strong understanding of pharmacology and therapeutics.
  • Excellent verbal and written communication skills.
  • The ability to provide timely and accurate recommendations regarding the safe and economical use of medications.
  • Proficiency in additional languages is an asset, with French being preferred.

Work Environment

This position primarily operates from a designated healthcare facility but may require flexibility to provide services across various locations as needed.

The hourly wage for this position is competitive and reflects the qualifications and experience of the successful candidate.
