Clinical Services Partner

Shannex is seeking a skilled Clinical Services Partner to join our Transitional Care team in Bedford, Nova Scotia. As a key member of our team, you will work collaboratively with healthcare professionals to provide high-quality patient care and support.
Key Responsibilities- Collaborate with the Transitional Care Team to support and care for individuals and their support networks during transitions within and across settings.
- Research and provide evidence-based recommendations for nurses and members of the Transitional Care Team to support safe and effective transitions in care.
- Work closely with Shannex and Nova Scotia Health to identify clinical education needs for employees and support the development of a clinical education plan.
- Deliver clinical and IPAC training sessions, workshops, and educational programs, and monitor their effectiveness.
- Participate in the development, evaluation, and revision of clinical policies, procedures, and education consistent with legislation, best practices, and Accreditation standards.
- Bachelor of Nursing or Bachelor of Science in Nursing degree.
- Current registration or eligibility to be registered with the appropriate nursing regulatory body.
- Minimum five years of nursing experience, including clinical, administrative, and leadership experience.
