Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Fraser Health, providing clinical pharmacy services to clients in Correctional Health Services locales.

Key Responsibilities
  • Provide comprehensive medication management and pharmaceutical care to clients, monitoring their medical condition(s) and ongoing drug therapy for effectiveness, adherence, adverse reactions, drug interactions, and therapeutic, chemical, and laboratory incompatibilities.
  • Communicate pertinent findings, recommendations, and plans to clients and healthcare practitioners, ensuring seamless care coordination.
  • Consult with clients and healthcare team members to develop client-specific therapeutic plans based on chart review, client information, and assessment of clinical responses.
  • Maintain accurate and up-to-date documentation in the health record, adhering to organizational standards and practices.
  • Utilize resources to support professional practice and client care, including provincial programs, academic detailing, optimal drug use initiatives, opioid stewardship, and de-prescribing.
  • Establish linkages with health authority, community pharmacy services, and other relevant community services to ensure continuity of care for clients.
  • Provide drug information, consultation, and support services to healthcare team members, communicating on drug-related issues, questions, and client-specific therapeutic regimens.
  • Conduct client counseling and education regarding drug therapy, providing in-service education to the healthcare team as needed.
  • Supervise students, such as University Pharmacy Undergraduates and Pharmacy Residents, and participate in various committees to provide guidance and advice on drug therapy.
  • Contribute to quality improvement activities by reviewing current practice, identifying actual or potential problems, and recommending and evaluating modifications.
  • Maintain workload records and metrics as required, participating in programs such as Patient Safety and Drug Reaction Reporting.
  • Share responsibility for orienting new personnel and act as a role model, maintaining a working knowledge of PharmaCare policies and procedures.
Qualifications
  • Current registration as a pharmacist with the College of Pharmacists of British Columbia.
  • Two years' recent, relevant patient care experience or equivalent combination of education, training, and experience.
  • Valid BC Driver's Licence and access to a personal vehicle for business-related purposes.
