Job Summary
Mackenzie Health is seeking a highly skilled Clinical Pharmacist to join our multidisciplinary team. As a Clinical Pharmacist, you will play a critical role in providing pharmaceutical care to patients, contributing to safe and quality care, and working collaboratively with healthcare professionals to achieve desired patient outcomes.
Key Responsibilities
- Assess, plan, implement, and evaluate drug therapy effectively to ensure optimal patient outcomes.
- Educate patients about medications to promote autonomy, reduce non-compliance, and prevent complications.
- Apply the pharmaceutical care model to identify, resolve, and prevent drug-related problems.
- Conduct drug utilization reviews to ensure cost-effective medication usage.
- Coordinate the acquisition and supply of in-center and home medications to ensure seamless care continuity.
- Participate in formulating the interdisciplinary plan of care.
- Provide effective supervision, leadership, and direction to Pharmacy Technicians, students, and volunteers.
- Contribute to a safe, inclusive environment for all by complying with patient and staff safety policies and procedures.
Requirements
- Bachelor's Degree in Pharmacy.
- Licensed with the Ontario College of Pharmacists in Part A of the Register (or eligible for registration).
- Previous hospital experience required.
Preferred Qualifications
- Hospital residency an asset.
- PharmD an asset.
- Demonstrated pharmaceutical care, communication, and computer skills.
