Clinical Resource Nurse

Position Overview: The Transitional Care Unit is a specialized 40-bed facility dedicated to assisting patients who have a confirmed discharge destination but encounter temporary challenges, such as awaiting home care services or rehabilitation. Our focus is on providing short-term, targeted support to facilitate quicker and safer discharges.

Team Composition: The healthcare team is composed of various professionals, including physicians, registered nurses, practical nurses, patient care assistants, dietitians, pharmacists, physiotherapists, occupational therapists, a discharge planner, social worker, respiratory therapist, speech language pathologist, chaplain, unit clerks, and volunteers. Each member plays a crucial role in ensuring optimal health outcomes and seamless transitions for our patients.

Role of the Resource Nurse: As the Resource Nurse, you will oversee the daily care delivery activities of the staff, ensuring adherence to the standards, policies, and procedures of the Health Centre and Unit.

Qualifications:

  • Current Certificate of Registration from the College of Nurses of Ontario
  • Baccalaureate preparation in a health-related field is preferred
  • Experience as a Resource Nurse, Team Leader, or Charge Nurse
  • Leadership, teaching, and/or mentorship experience
  • Current Basic Life Support certification
  • Completion of a recognized Leadership Diploma/Certificate or equivalent is preferred
  • Canadian Nurses Association Specialty Certification relevant to the nursing service area
  • Continuing education courses specific to the nursing service area
  • Progressive clinical experience
  • Proficiency in current software applications
  • Membership in a professional association relevant to the nursing service area
