Job Title: Nurse Practitioner

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Barrie and Community Family Health Team.

Job Summary:

The successful candidate will provide high-quality patient care, conduct health assessments, and develop treatment plans in collaboration with our healthcare team.

Responsibilities:
  • Conduct thorough health assessments and develop individualized treatment plans
  • Provide routine check-ups, vaccinations, and health education to patients
  • Collaborate with our healthcare team to coordinate patient care
  • Order and interpret diagnostic tests, prescribe medications, and manage chronic conditions
  • Participate in quality improvement initiatives and contribute to the development of our healthcare team
Requirements:
  • Master's degree in Nursing or related field
  • Current registration with the College of Nurses of Ontario
  • Experience in primary care or related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
