Job Title: Transitional Support Services Worker
Job Summary:
We are seeking a skilled Transitional Support Services Worker to join our team at the Canadian Red Cross. As a key member of our team, you will provide essential support to clients during their transition from hospital to home, ensuring a smooth and safe transition.
Key Responsibilities:
- Provide support to clients during hospital discharge and transition to home, following established service plans.
- Complete basic errands and home management duties, such as preparing nutritious meals and maintaining a safe environment.
- Conduct risk assessments of clients' homes using standardized tools.
- Facilitate client and caregiver engagement, referrals to community support services, and discharge processes.
- Identify and report any safety concerns or hazards.
- Provide companionship to clients within therapeutic boundaries.
- Maintain confidentiality and adhere to program guidelines.
Requirements:
- Standard First Aid and CPR-C certification.
- Fluency in English (French is an asset).
- Previous experience working with elderly clients is an asset.
- Excellent interpersonal skills, including dependability, punctuality, and stress management.
- Ability to follow care plans and work in an environment with ambiguity.
- Proven analytical and problem-solving skills.
Working Conditions:
- Primarily conducted in clients' residences.
- Hours of operation: 8:30 am to 4:30 pm, Monday to Friday.
- Irregular hours may be required, but never outside of regular hours.
