Clinical Supervisor

About the Role:
We are seeking a skilled Clinical Supervisor to join our team at ParaMed Home Health Care. As a Clinical Supervisor, you will be responsible for supervising and supporting our Personal Support Workers (PSWs) and Health Care Aides (HCAs) to ensure they provide high-quality care to our clients in the community.
Key Responsibilities:
- Supervise and support PSWs and HCAs to ensure they comply with legislated standards of practice and company policies.
- Respond to and resolve concerns from community staff, patients, and families.
- Provide ongoing leadership through regular huddles, face-to-face meetings, and one-on-ones.
- Initiate patient safety incident investigations and document to ensure timelines and action plans are met.
- Work collaboratively with nursing team members to support clinical documentation and submission requirements.
- Engage patients in problem-solving using a collaborative approach, offering health teaching, building rapport, and trust to identify patient healthcare needs, strengths, capacities, and goals.
- Engage staff in problem-solving, offering education, coaching, and mentoring in resolution of problems.
- Maintain knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice.
Requirements:
- Current RN or RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions.
- Minimum of 5 years' experience as a practicing Registered Nurse or Registered Practical Nurse.
- Comprehensive patient health assessment skills are required.
- Possess an understanding of the principles of community nursing practice.
