Job Summary:
VON Canada is seeking a skilled Registered Nurse to join our team as a Community Health Specialist. As a key member of our healthcare team, you will provide medical care to children, youth, adults, and seniors in their homes and other settings in communities across Nova Scotia.
Key Responsibilities:
- Provide medical care to clients in their homes and other settings, including diabetes management, dialysis, IV medications, and wound care.
- Work collaboratively with other healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments of clients' physical and emotional needs, and provide education and support to promote healthy living.
- Collaborate with community resources and healthcare providers to ensure seamless care coordination.
- Document client interactions and care provided in a timely and accurate manner.
Requirements:
- Current registration with the College of Registered Nurses of Nova Scotia.
- Experience in community health an asset.
- Valid driver's license and access to a reliable vehicle.
- Current and original copy of a satisfactory Criminal Records Check.
- Ability to wear Personal Protective Equipment (PPE).
Working Conditions and Physical Demands:
This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
