Job Summary

We are seeking an experienced Licensed Practical Nurse (LPN) to lead our Home Support Services team. As a Home Support Team Leader, you will be responsible for coordinating and supervising Community Health Workers, overseeing the implementation of client care plans, and providing training and evaluation of personal care techniques.

Key Responsibilities
  • Coordinate and supervise Community Health Workers to ensure effective delivery of home support services
  • Oversee the implementation of client care plans to ensure high-quality care and services
  • Provide training and evaluation of personal care techniques to ensure staff are equipped to provide competent care
  • Advocate for clients and their families to ensure their needs are met
  • Collaborate with interprofessional teams to provide comprehensive care and services
Requirements
  • Registration with BC College of Nurses and Midwives as a practicing LPN registrant
  • Two years of recent, related experience, including one year in a supervisory role
  • Current valid BC Driver's License
