As a Community Physiotherapist with Lifemark, you will play a vital role in enhancing the quality of life for clients in the Durham Region. About Lifemark:
As the largest community-based rehabilitation provider in Durham Region, we have been dedicated to delivering high-quality rehabilitation services for both adults and children since 1997. Our mission is to be recognized as a trusted healthcare provider, helping clients maximize their functional abilities and engage fully in life.
Key Responsibilities:
- Deliver ministry-funded physiotherapy services in clients' homes.
- Conduct thorough physiotherapy assessments using various techniques, focusing on geriatrics, neurology, orthopedics, and palliative care.
- Collaborate with clients and caregivers to develop and implement personalized treatment plans.
- Supervise physiotherapy assistants and ensure quality care delivery.
- Educate clients and caregivers, connecting them with community resources as needed.
- Maintain accurate documentation of assessments, care plans, and progress in accordance with professional standards.
Qualifications:
- Registered Physiotherapist with the College of Physiotherapists of Ontario.
- Experience in community settings is preferred, but new graduates are encouraged to apply.
- Ability to work independently and collaboratively within an inter-professional team.
- Strong organizational skills and the ability to manage self-scheduling.
- Excellent communication skills, both oral and written.
- Proficient in computer usage.
- Reliable transportation is essential for this role.
