Job Summary

The Horizon Health Network is seeking a highly skilled and dedicated Primary Health Care Nurse Practitioner to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality care to patients in a variety of settings.

Key Responsibilities

  • Provide comprehensive health assessments and develop individualized care plans for patients.
  • Order and interpret diagnostic tests, prescribe medications, and provide treatment as needed.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Participate in quality improvement initiatives and contribute to the development of evidence-based practices.
  • Act as a consultant to other healthcare professionals and provide education and support as needed.

Requirements

  • Active registration as a nurse practitioner with the Nurses Association of New Brunswick.
  • Minimum of 1 year of clinical practice experience as a NP within the last 5 years.
  • Ability to work independently and in teams; to lead and drive change.
  • Exemplary written and verbal communication skills in English.
  • Demonstrated ability to attend work on a regular basis.
