About the Role

We are seeking a skilled and compassionate Community Health Nurse to join our team. In this role, you will provide exceptional nursing services to patients in their homes, clinics, and other community settings. Your expertise will be valued as you work collaboratively with patients, families, and healthcare teams to develop and implement effective care plans.

Key Responsibilities
  • Provide nursing services to patients in their homes, clinics, and other community settings
  • Develop and implement effective care plans in collaboration with patients, families, and healthcare teams
  • Communicate and liaise with patients, families, and healthcare teams to ensure seamless care delivery
  • Work independently or in cooperation with the Home and Community Care Support Services (HCCSS) to provide comprehensive care
  • Participate in ongoing education and training to enhance your skills and knowledge
