Job Overview

We are seeking a compassionate and skilled Registered Dietitian to join our Home Support team on a part-time basis. In this role, you will have the opportunity to make a real difference in the lives of patients by providing personalized nutrition counseling and support.

Responsibilities

  • Conduct comprehensive nutritional assessments of patients, taking into account their medical history, dietary habits, and lifestyle factors.
  • Develop individualized nutrition care plans to address specific patient needs, including those with dysphagia, malnutrition, or enteral nutrition requirements.
  • Monitor patient progress and make adjustments to their nutrition plan as needed.
  • Collaborate effectively with healthcare professionals, families, and caregivers to ensure seamless patient care.
  • Provide education and counseling to patients on healthy eating habits, food safety, and disease management through nutrition.
  • Conduct home visits to assess patients' nutritional needs and provide support in their homes.
  • Identify resources and connect patients with appropriate community services.

Qualifications

  • Bachelor's degree in Dietetics or a related field.
  • Registration with the Professional Order of Dietitians of Quebec.
  • Experience working with hospitalized patients is an asset.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Proficiency in Microsoft Office Suite and relevant healthcare software.
