Job Overview
We are seeking a compassionate and skilled Registered Dietitian to join our Home Support team on a part-time basis. In this role, you will have the opportunity to make a real difference in the lives of patients by providing personalized nutrition counseling and support.
Responsibilities
- Conduct comprehensive nutritional assessments of patients, taking into account their medical history, dietary habits, and lifestyle factors.
- Develop individualized nutrition care plans to address specific patient needs, including those with dysphagia, malnutrition, or enteral nutrition requirements.
- Monitor patient progress and make adjustments to their nutrition plan as needed.
- Collaborate effectively with healthcare professionals, families, and caregivers to ensure seamless patient care.
- Provide education and counseling to patients on healthy eating habits, food safety, and disease management through nutrition.
- Conduct home visits to assess patients' nutritional needs and provide support in their homes.
- Identify resources and connect patients with appropriate community services.
Qualifications
- Bachelor's degree in Dietetics or a related field.
- Registration with the Professional Order of Dietitians of Quebec.
- Experience working with hospitalized patients is an asset.
- Excellent communication, interpersonal, and problem-solving skills.
- Proficiency in Microsoft Office Suite and relevant healthcare software.
