Physiotherapist - Rehabilitation Specialist

We are seeking a highly skilled Physiotherapist to join our dynamic Rehabilitation Team at CarePartners. As a key member of our team, you will play a critical role in providing high-quality patient care and contributing to the growth and development of our therapy/rehabilitation program.
Key Responsibilities- Provide professional assessment and treatment of patients in the community and in their homes
- Develop and maintain effective care plans in collaboration with Community Health Services and Home and Community Care Support Services (HCCSS)
- Provide guidance on workplace health and safety
- Collaborate with the Safety, Health, and Wellness team and Pandemic Response team to ensure a safe and healthy work environment
- Conduct thorough assessments and develop personalized care plans to promote independence and improve patient outcomes
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
- Master's Degree (or equivalent) in Physiotherapy from a recognized university
- Proven computer skills, including proficiency in email, word processing, and work scheduling systems
- Excellent English verbal and written communication skills
- French language skills are an asset
