Patient Care Coordinator

About the Role:
We are seeking a skilled and compassionate Registered Nurse to join our team as a Nurse Care Specialist. In this role, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities:
- Conduct thorough assessments of patients' individualized needs and develop personalized care plans.
- Execute and continuously monitor care plans to ensure optimal patient outcomes.
- Build strong relationships with patients and their families through a collaborative and empathetic approach.
- Maintain accurate and up-to-date professional patient records related to patient encounters and plans.
Requirements:
- Registration with the CNO as a Registered Nurse in good standing.
- Valid driver's license and access to a vehicle.
- Experience in delivering community care is an asset.
- Reliability and flexibility.
- Willingness to travel to and from patient residences within a designated area.
- Passion for making a difference in your community.
- Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs.
