About the Role:
We are seeking a skilled Registered Practical Nurse to join our team as a Community Care Specialist. In this role, you will provide high-quality nursing care to patients in their homes, working collaboratively with our team to deliver exceptional patient outcomes.
Key Responsibilities:
- Participate in scheduled visits in the community to assess and address patient needs within your scope of practice.
- Provide safe and effective nursing care, using appropriate assessment tools and techniques to identify individual patient needs.
- Develop and implement personalized care plans, monitoring patient progress and adjusting plans as required.
- Build strong relationships with patients and their families, promoting trust and confidence in our care services.
- Maintain accurate and up-to-date patient records, documenting all interactions and care provided.
Requirements:
- Registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a reliable vehicle.
- Experience in delivering community care is an asset, but not required.
- Excellent communication and interpersonal skills, with a strong focus on patient-centered care.
