Position Overview: We are currently looking for a passionate and skilled Nurse Practitioner to become an integral part of our dynamic interprofessional team, specifically focusing on our geriatric patient demographic. This role involves leveraging our cutting-edge virtual care platform to foster innovation and enhance client outcomes. Evening and overnight on-call responsibilities will be part of this position.

Key Responsibilities:

  • Conduct thorough and advanced health evaluations.
  • Utilize evidence-based methodologies to establish diagnoses and develop comprehensive, holistic care strategies.
  • Demonstrate proficiency in virtual care technologies and adapt to rapid advancements introduced by our technical team.
  • Deliver clinical services through a virtual platform and secure consent for treatment.
  • Diagnose medical conditions and implement interventions within the Nurse Practitioner scope of practice.
  • Order and interpret diagnostic tests and prescribe necessary medications.
  • Manage pain and symptoms associated with specific diseases.
  • Provide follow-up care for clients whose initial consultations may have been initiated by another Nurse Practitioner or Medical Doctor.
  • Assess and address the educational needs of clients, their families, and informal caregivers.
  • Offer education, coaching, and counseling for both acute and chronic health issues.
  • Engage in regular business meetings to contribute to program development and ongoing evaluation.
  • Be available for on-call and after-hours services for Rhynocare clients.
  • Exhibit leadership and serve as a role model in critical thinking, ethical decision-making, and evidence-based service planning.

Qualifications:

  • Completion of an accredited Nurse Practitioner program.
  • Preference for candidates with a Primary Health Care Nurse Practitioner designation.
  • Current registration with the College of Nurses of Ontario in the Extended Class.
  • A minimum of 3 years of nursing experience, including at least 2 years as a Nurse Practitioner.
  • BLS-HCP certification.
  • Proficiency in a second language is considered an asset.
  • Access to a home office is required.
