Compensation Overview

The salary for this position ranges from $67,867 to $97,550, determined by the applicant's qualifications, experience, and an internal equity review.

Your Role in Making a Difference

The Indigenous Patient Navigator (IPN) plays a vital role within the Interdisciplinary Care Team, serving as a resource for Indigenous clients and healthcare providers. The IPN ensures that care is culturally safe and centered around the client's needs, working across various health sectors, including acute care, mental health, and public health.

Key Responsibilities
  • Offer consultative and navigational services to health professionals and clients, assisting in understanding client needs through cultural interpretation and health literacy support.
  • Facilitate discharge planning by collaborating with clients and healthcare providers to identify needs and connect with appropriate community resources.
  • Act as a liaison between Indigenous communities and healthcare providers, enhancing understanding of the healthcare system and available services.
  • Coordinate meetings among the inter-professional team to address concerns and barriers faced by clients.
  • Adhere to Occupational Health and Safety policies, reporting any hazards or unsafe conditions promptly.
